BEFORE You Download the Forms:

  1. Make sure you have access to both:
    • Netscape for Windows or Macintosh, version 1.1N or higher
    • MS Word for Windows version 6 or higher
  2. Review the instructions for downloading and saving the forms.

If You Experience Technical Problems with Downloading the Forms:

  • Contact your local computer support person. Most likely, they will be able to assist you at the moment you are experiencing the problem.
  • If you are not able to get local computer support, e-mail ORA_IT_Request (oraitrequest@indiana.edu) and include your telephone number and an explanation of the problem. He will contact you as quickly as possible.

If You Want to Receive a Paper Copy of the Forms, Or
Have Questions/Problems Using the Forms After They Have Been Downloaded:

  • E-mail OHRPP staff (Senta K. Baker, Ryan T. Merckle, and Sharon Nejfelt) (iub_hsc@indiana.edu). Include your address to receive a paper copy of the forms. Otherwise, for questions and problems, include your telephone number and explain your question or problem.

Instructions to Download and Save the Forms

Use these instructions for both Windows and Macintosh versions of Netscape. Only the first step requires you to do something different if you're in Windows or on a Macintosh.

  1. Netscape (or Explorer) -Windows: hold down the right mouse button over the Application Packet, Continuing Review Form, or Dissertation Form links below.

    2. Netscape-Macintosh: hold down the mouse button over the Application Packet, Continuing Review Form,or Dissertation Form links below.
  2. Select Save Link Target As... (in Netscape) from the menu that appears to the right of the link.  (In Explorer select Save Target As...)
  3. You see a typical Save window. By default, the files are saved as "formswd.doc" if you selected the Application Packet, as "continuingwd.doc" for the Continuing Review Form, or as "disserta_fm.doc" for the Dissertation Form. You only need to specify where you will save the file. So, be sure to choose the drive (C: for hard disk, or A: for floppy disk) and directory in which you want to save the file.
  4. Click OK or Save and the file will be saved.
  5. When the download is complete, quit your browser.
  6. Start MS Word, then open and print the downloaded file. You may need to re-format the file
